Questions

Your primary care home and the concierge experience

Helix Advanced Medicine is built to be your medical home, not a specialty boutique you visit once. You can receive full primary care here, then layer on a concierge experience when you want more time and faster access. We also excel in offering coordinated plans for weight loss, hormone replacement, brain health, skin, and heart disease/cancer prevention. The questions below explain how that works.

Your primary care home and the concierge experience

What is Helix Advanced Medicine?+

Helix Advanced Medicine is a physician-led practice that serves as a primary care home and offers a concierge experience on top of that foundation. We take care of everyday medicine including acute illness, chronic conditions, prevention, and coordination, and we also provide advanced services in metabolic health, medical weight management, hormone optimization, brain health, in-office diagnostics, medical aesthetics, and skin care. One practice. One record. A team that already knows you.

What does “primary care home” mean here?+

It means Helix can be your regular doctor’s office. We manage annual and periodic preventive visits, blood pressure and cholesterol, diabetes and prediabetes, thyroid disease, infections, minor injuries, medication refills, specialist referrals, hospital and imaging follow up, and the work of keeping your chart coherent. You do not have to keep a separate regular primary care physician unless you prefer to. Continuity is the point. The clinician who treats your sinus infection is the same one watching your memory, weight, hormones, and skin over years rather than visits.

How is the concierge experience different from ordinary primary care?+

Traditional primary care is built around short visits and crowded panels. The concierge layer at Helix is built around time and access including longer appointments, same day or next day visits when medically appropriate, direct communication with your care team, unhurried annual exams, and a plan that includes prevention and optimization rather than only problem lists. You are not buying a luxury lobby. You are buying a smaller panel and a practice designed to notice change early.

Do I have to join a concierge membership to be a patient?+

No. Many patients use Helix as a primary care home with standard visits. The concierge membership is for patients who want enhanced access, longer visits, proactive check ins, and bundled advanced services. You can start with primary care and add concierge later or join the membership from day one. We will explain what is included in each path, what is billed separately, and what insurance may cover before you decide.

Can Helix replace my current primary care physician?+

Yes, if you want it to. We can assume first call primary care, keep your preventive calendar, manage chronic disease, and coordinate specialists. If you already have a primary care physician you like, we can work alongside that relationship for a defined set of services such as weight management, hormones, brain health, or aesthetics. Split care only works when communication is explicit. We will not silently duplicate or contradict another clinician’s plan.

What does a typical first year look like if Helix is my medical home?+

A comprehensive intake and baseline exam, in-office or coordinated diagnostics, a written plan for prevention, chronic conditions, and any advanced programs you choose including weight, hormones, brain health, or skin, then scheduled follow up plus access when something acute comes up. Concierge members have a defined communication path between visits. The goal is that nothing important waits six months for the next physical.

Do you see children or only adults?+

We focus on adult primary care and adult advanced services. We can advise on whether a pediatric or family medicine colleague is the better medical home for children in your family.

Who provides the care?+

Care is provided by Kevin Neese MD and Allison Snyder NP. They have a combined forty five years of medical experience and have worked together for the past fifteen years. Primary care, concierge visits, and advanced programs are delivered by licensed clinicians with specialized training. Procedures and prescribing follow scope of practice and collaborative practice rules in Indiana.

In-office diagnostics

What testing can be done in the office instead of sending me out?+

Whenever it is accurate and appropriate, we prefer to test here so results fold into the same visit and the same plan. In-office capabilities include phlebotomy and point of care labs, twelve lead electrocardiograms, body composition analysis, cognitive screening, targeted ultrasound or point of care ultrasound, pulmonary function testing, and VO2 max testing.

Why does in-office diagnostics matter?+

Sending every test to a hospital lab or imaging center adds delay, extra appointments, and fragmented results. Doing the right tests on site lets us explain findings while you are still in the room, adjust a plan the same day, and keep one medical home accountable for follow up. When a study truly needs a hospital or specialty center such as MRI, CT, formal echocardiography, or sleep study, we arrange it and close the loop. We have close relationships with our referral imaging centers and specialty care and will coordinate those visits for you.

Do you offer point of care ultrasound?+

Yes. Point of care ultrasound is used as an extension of the exam, for example to evaluate selected soft tissue, vascular, cardiac, or abdominal questions in real time, not as a replacement for a formal radiology study when one is indicated. Findings are documented in your chart and acted on.

Do you measure body composition or only weight?+

Weight alone is a weak vital sign. When clinically useful we measure body composition so we can track fat mass, lean mass, and visceral fat, especially during GLP 1 therapy, hormone optimization, or a strength focused plan. That is how we judge whether you are losing the right tissue.

How do you handle labs?+

Many labs can be drawn in the office. Others go to a reference laboratory. We order what the question requires rather than a ninety marker panel for its own sake, and we review results with you rather than leaving them in a portal. Repeat testing is timed to treatment such as GLP 1 titration, hormone dosing, or cognitive workup, not to a billing calendar.

Will you use my previous records and outside tests?+

Yes. Bring prior labs, imaging, hospital summaries, and medication lists or authorize us to obtain them. Repeating a test you already had last month is waste. Missing a result that changes the plan is worse.

Brain health screening, evaluation, and treatment

What is the Brain health program?+

Brain health at Helix is primary care based screening, evaluation, and ongoing management of memory, thinking, and cognitive risk delivered inside your medical home instead of as a one off memory test. We look for change early, search for causes that can be treated, manage what we can manage here, and coordinate neurology, imaging, and community resources when a specialist or a disease modifying therapy is the right next step. The work is longitudinal. A single screening score is not a diagnosis.

Who should consider brain health screening?+

Adults with new forgetfulness, word finding trouble, getting lost, personality change, or a family member who is worried. Adults with a strong family history of dementia. Adults who simply want a baseline while they still feel well. Screening is also reasonable as part of a thorough midlife or later life preventive exam. Worry alone is not a diagnosis and neither is “it is just aging” without a look.

What does a brain health evaluation include?+

A detailed history from you and when possible someone who knows you well, medication and alcohol review, mood and sleep assessment, hearing and vascular risk review, a validated cognitive screen and more formal testing when indicated, targeted labs that can uncover reversible contributors such as B12, thyroid, metabolic and selected nutrient or inflammatory markers, and imaging or specialty referral when the picture warrants it. We establish a baseline so the next visit is a comparison rather than a guess.

Can memory problems be caused by something other than dementia?+

Often yes. Treatable or contributory factors include medication effects, sleep apnea, hearing loss, depression and anxiety, thyroid disease, B12 deficiency, alcohol, uncontrolled vascular risk, vision problems, and metabolic disease. Finding those is one of the highest yield parts of a primary care brain health visit. We do not jump to Alzheimer’s because a screening test was low.

Do you diagnose and treat dementia at Helix?+

We evaluate cognitive concerns, identify mild cognitive impairment versus a dementia syndrome when the data support it, treat reversible contributors, optimize vascular and metabolic risk, support caregivers, and manage the day to day medical care of patients living with dementia including symptom directed medications when appropriate, safety planning, and coordination. Formal diagnosis of a specific neurodegenerative disease sometimes requires neurology, neuropsychology, and imaging we will arrange. Newer disease modifying therapies for Alzheimer’s disease, when they apply, are considered in partnership with specialty colleagues. We will be clear about what we can do here and what we should not overclaim.

Is there a treatment that reverses Alzheimer’s disease?+

There is no honest guarantee of reversal. Some patients benefit from treating reversible causes and tightening vascular risk, sleep, hearing, movement, and social engagement. It is estimated that up to forty five percent of dementia is attributed to modifiable risk factors. Selected patients may be candidates for newer anti amyloid or other specialty therapies after a full workup. Lifestyle and risk factor work is not alternative medicine. It is the foundation. We will not sell a protocol as a cure but address genetic predilection and modifiable risks to lower your risk of developing dementia.

How is brain health connected to the rest of my care at Helix?+

Intimately. Blood pressure, diabetes, weight, hearing, sleep, hormones, mood, and medications all sit in the same chart as your cognitive screen. That is the advantage of a primary care home. A neurology only visit cannot manage your GLP 1, your blood pressure, and your caregiver’s questions in one relationship. We can and we will pull in specialists when the problem leaves our lane.

Do you involve family members or caregivers?+

With your permission, yes. Cognitive care is safer and more accurate when someone who sees daily function is in the room or on the call. We can also discuss driving, finances, home safety, and community resources when those conversations are needed.

Aesthetic procedures and skin care

Do you offer aesthetic procedures?+

Yes. Medical aesthetics at Helix sit inside a primary care practice, not a separate spa with no chart. After a face to face evaluation we may offer neuromodulators such as Botox or similar, dermal fillers and biostimulators, chemical peels, and other in-office procedures that match our training and expertise. Every procedure has risks, limits, and a recovery course we will review before you book.

Why have aesthetics in a primary care and concierge practice?+

Your face and skin are part of the same patient as your blood pressure and your memory. Doing aesthetics here means the clinician already knows your medications including GLP 1s and hormones, autoimmune history, bleeding risk, and prior reactions. It also means we will decline a treatment that is fashionable but wrong for you.

What skin care services do you provide?+

Medical skin care includes evaluation and treatment of common concerns including acne, rosacea, eczema, pigment change, sun damage, and aging skin, and a plan that may combine prescription topicals, in-office procedures, and medical grade products. We also treat everyday dermatologic problems that belong in primary care including rashes, infections, and lesions that need examination or biopsy referral.

Do you sell skin care products?+

Yes. We have carefully developed a skin care line called Nazare that offers affordable clinical grade skin care and luxury. These include high grade topical Vitamin C, topical peptides, luxury eye care, topical stem cell creams, and retinoids. We may recommend or dispense selected medical grade products when they fit the plan. You are never required to buy products from us to receive care.

Are aesthetic treatments included in concierge membership?+

For those who wish to include aesthetic treatments, there is a concierge membership that includes discounted services. Typically, evaluation and medical decision making live inside the visit model. Injectables, devices, and product are billed separately so members who do not want aesthetics are not subsidizing those who do. Some memberships include a defined aesthetic credit. We will put numbers in writing before you schedule.

Who is not a candidate for injectables or energy devices?+

Pregnancy, certain neuromuscular conditions, active infection at the site, unrealistic expectations, recent procedures in the same area, and some medications or medical histories. GLP 1 related facial volume change is discussed honestly. More product is not always the answer. Sometimes the better plan is timing, weight stability, and skin quality work. We will say no when no is the safer sentence.

Medical weight management and hormone optimization

Do you offer GLP 1 weight management programs?+

Yes. Medically supervised GLP 1, 2, and 3 therapies remain a core advanced service now delivered inside a primary care home so your weight plan is not disconnected from your heart, brain, hormones, and muscle. Candidacy follows a proper evaluation generally a BMI of thirty or higher, or twenty seven or higher with a weight related condition, subject to exclusions such as certain thyroid cancer histories, prior pancreatitis, pregnancy, and significant gastrointestinal disease.

Do you use brand name or compounded GLP 1s?+

We review FDA approved products first. When a compounded formulation from a licensed compounding pharmacy is clinically appropriate and legally available, your clinician will explain differences and quality considerations. We do not prescribe without an evaluation and we do not use unlicensed online sources.

How much weight can I expect to lose?+

Results vary. Clinical trials of FDA approved GLP 1 and dual agonist medications have shown average body weight reductions in the mid teens to low twenties percent range with lifestyle support. We track body composition, not only the scale, and we plan for muscle preservation from day one.

Do you offer bioidentical hormone therapy and pellets?+

Yes, for appropriate patients after symptoms, history, and labs. Pellets are one delivery option placed in a brief in-office procedure and typically lasting several months. We will recommend a different route or no hormone therapy when that is safer.

How do weight, hormones, skin, and brain health connect?+

They share the same patient. Rapid weight loss changes face volume and skin. Hormones affect mood, sleep, muscle, and cognition. Vascular and metabolic disease are the largest modifiable drivers of later life brain decline. Treating these in four different offices is how things get missed. Treating them in one medical home is the design.

How care works

How do I get started?+

Schedule a meet and greet or a comprehensive initial visit and tell us whether you want Helix as your primary care home, a concierge membership, or a defined program such as weight, hormones, brain health, or aesthetics. We will strive to answer any questions you may have before making a decision. Bring prior records if you have them. We will map services, costs, and follow up before you commit.

Do you offer telehealth?+

Yes, for visits that can be done well by video and for patients in states where we are licensed. Many acute issues, medication follow ups, and care plan reviews work by telehealth. Procedures, certain exams, diagnostics, pellet insertion, and most aesthetics are in person.

How quickly can I be seen when I am sick?+

Concierge members have priority access for same day or next day visits when medically appropriate. Primary care home patients are offered the first clinically suitable opening. We will publish a real access standard rather than a slogan.

Where are you located and what are your hours?+

We are located at 2020 South State Road 135 Suite 200 Greenwood Indiana 46143 adjacent to Dye’s Walk golf course. Office hours are eight in the morning to four thirty in the afternoon. There is ample parking both in front and in the rear of the building.

Cost, insurance, and membership

Do you accept insurance?+

Primary care services that are medically necessary and billable may be submitted to insurance when we are in network or when you want a superbill. Concierge membership fees, many advanced wellness services, most aesthetics, and some diagnostics are cash pay because they are not covered or not covered well by traditional plans. We separate those categories so the bill is understandable.

What does concierge membership include?+

A typical membership includes enhanced access, longer visits, a comprehensive annual exam, defined communication standards, and care coordination. Medications, injectables, devices, some labs, and hospital care are usually extra. Membership is not a substitute for health insurance.

Do you accept HSA or FSA funds?+

Many patients use HSA or FSA funds for qualifying medical services. Eligibility depends on your plan administrator and the specific service. We provide itemized receipts.

If I only want aesthetics or a GLP 1 program, do I still need to be a primary care patient?+

Not always. We can see you for a defined program. We will still take a proper history because those treatments are medical. If you already have a primary care physician, we will ask to coordinate. If you do not, we will offer to become that home.

Safety, privacy, and expectations

Is this a medical practice or a med spa?+

A medical practice that also offers aesthetics and skin care. Prescribing, diagnostics, dementia evaluation, and procedures are performed by licensed clinicians after an appropriate evaluation. The spa elements do not run the medicine.

Will my information stay private?+

Yes. We follow HIPAA. Telehealth uses a secure platform. We do not sell your data. Family involvement in brain health visits happens only with your permission or as otherwise allowed by law.

Can you guarantee results including weight, memory, or skin?+

No. We guarantee a thorough evaluation, honest limits, close follow up, and a plan that lives in one chart. Biology, adherence, and time still decide outcomes.
